HSA Dollars and a Doctor With Time: What Changed in 2026

If you have ever left an appointment with half your questions still in your purse, this one is for you.
The short version
As of January 1, 2026, membership at Stanford Clinic is HSA-eligible. Pre-tax dollars you have already set aside can go toward your monthly membership.
Where that comes from
The One Big Beautiful Bill Act, signed in July 2025, created a new category in the tax code for direct primary care arrangements. Beginning January 1, 2026, an arrangement that meets the federal definition counts as a qualified medical expense, and no longer knocks a patient out of contributing to an HSA the way the old rules could.
Not every practice's arrangement meets that definition - it has to cover primary care, and the fee has to stay under the federal thresholds of $150 a month for an individual or $300 for an arrangement covering more than one person. Stanford Clinic's membership is structured to meet those requirements, which is why we can say plainly that membership here is HSA-eligible.
Your side of it
Those thresholds describe the clinic's arrangement, not a limit on you. What varies from family to family is the HSA itself - whether you have one, and whether your health plan qualifies you to use and fund it. That is a question for your plan administrator, and it is worth asking before you assume either way.
Why the tax change is not the best part
Here is what mamas around here actually notice: direct primary care changes the pace of the visit. No stopwatch. No feeling like you are keeping somebody from the next room. Time enough to finish the sentence, ask the second question, and bring up the thing you almost talked yourself out of mentioning.
That is the model Dr. Casey built Stanford Clinic around - a doctor who knows your family, in the same small town you buy groceries in.
How to ask
If you are curious how membership works with your HSA, call the clinic at (601) 947-0250 and ask about a free 10-minute fit call. It is a short conversation, no pressure - just a chance to hear how membership works and decide whether it suits your family. Please call; that line does not take texts. Bring your plan questions, and your HSA administrator can confirm the rest.
